Supporting materials for Final Write-up v1 should be submitted as attached files here. This makes it easier and faster to upload/download files.

There are a lot of submitted materials so follow these format rules:

• Clearly identify the content by assigning descriptive titles such as Interview Consent or Focus Group Coding etc.
• If materials were created in Word (or similar) submit as Word docs (.doc/.docx).
• If materials were created in other formats such as Excel or are photos of consent forms convert them to PDF or insert into a Word doc.
• If you (for some reason) coded data (or did anything) in longhand, transcribe into a word doc. NO scans or photos of your handwritten materials accepted. Seriously, it's the 21st century!
• Merge consent forms into ONE document for each for focus group(s) and interviews. All scans or photos must be legible. 

The sole purpose of this assignment page is for convenience and to verify that the basic requirements for Final Write-up v1 are met. There are no points attached to this specific assignment. Points earned or lost are reflected in your Final Write-up v1 grade.

IMPORTANT: In order to meet specific assignment requirements such as methods, you must include these supporting materials. For interviews and focus group participants to "count" you need signed consent forms.

• Interview questions with probes only (no transcripts or recordings needed).
• Surveys as they were administered (no complete surveys needed).
• Focus group questions/activities and related materials.
• Completed (filled out) coding sheets for each method (3 minimum).
• Signed consent forms (CLEARLY scanned or photographed, merged, and converted to pdf).

MISSING MATERIALS IS THE #1 MOST COSTLY ERROR AS THEY HAVE FIXED VALUES (EG. NO CODING SHEETS= -10 POINTS). CLEARLY IDENTIFY MATERIALS, MERGE AS DIRECTED, AND USE THIS AS A CHECKLIST!
